Biotech Intelligence Briefing · 2029–2042 · Viking Therapeutics · VKTX · VK2735

An early lead on small numbers — and one filing behind Lilly.

At matched weeks 20–36 VK2735 is ~3–5 pp ahead of tirzepatide, but on 13–118 patients in an exploratory study; VANQUISH at week 78 decides. Its commercial edge is monthly maintenance, and retatrutide files first. [gf3_ind_17p5, gf2_tzp15_w20, gf3_control_w33, gf2_tzp15_w33, gf3_ind_pools, gf3_registry, rt_bla, gf3_pipeline]

The thesis, in one picture

Risked VK2735 revenue for all three presets, the active one highlighted, against retatrutide's share of the treated pool and the class net-price curve.

VK2735 17.5 mg took off 17.8% of body weight by week 21 and 21.7% by week 33, against ~12.6% and ~17.5% for tirzepatide 15 mg at the same weeks: a real early lead, but one resting on a pooled cohort of 118, a week-33 arm of 13 and one placebo arm of 15, in an exploratory, unregistered endpoint [gf3_ind_17p5, gf3_control_w33, gf2_tzp15_w20, gf2_tzp15_w33, gf3_ind_pools, gf3_registry]. Retatrutide (−28.3% at week 80, efficacy estimand [rt_t1_wt]) files in Q1 2027 [rt_bla] while VANQUISH reads out in 2H27 [gf3_pipeline], so the base case prices VK2735 as a late weekly entrant with a monthly tail: 5% of the non-retatrutide pool [A], $1.71B unrisked and $1.38B risked 2034 revenue (§7). The bull case needs VANQUISH to confirm the lead and monthly dosing to command a price, as Pfizer's Metsera CVRs (up to $20.65 a share, ~$2.17B in aggregate, all tied to ~28-day dosing) suggest it can [nw_mtsr_final, nw_mtsr_cvr_usd].

A weekly dual priced into an eroding class, one filing behind

Base risked 2034 revenue $1.38B (§7) under a class net price that steps down at semaglutide's 2032 patent expiry [mk29] and again at tirzepatide's on 2036-05-13 [gf1_ob_780]. After 2036 a generic tirzepatide sits at the top of the efficacy range at commodity prices. That caps VK2735's terminal value more than its own ~2042 expiry does [vk_ip_family]. Axes: risked revenue $M (left) · retatrutide share of the US treated pool (right) · VK2735 SC blended net $/month (far right).

Competitive landscape — monotherapy, like for like

Every cell carries a provenance tier and its estimand. Toggle EE / TRE: cells on the other estimand grey out; cells whose estimand is not stated stay hatched in both modes.

Sixteen rows (berobenatide weekly and monthly shown separately), monotherapy at the dose taken forward, like for like. Weight loss is split by estimand: toggle EE (on-treatment; Viking's efficacy-population MMRM sits here) or TRE (treatment-regimen, ITT-like, what FDA generally asks for [gf4_fda_estimand]). Cells whose estimand is not stated (VESPER-1 berobenatide, amycretin, pemvidutide, enicepatide in T2D) are hatched in both modes [nw_bero_v1, nw_zena_sc_p1b, ms18, nw_ct388_t2d]. Durations are not normalised: VK2735 has 13 to 33 weeks [vk_venture_design, gf3_ind_17p5, gf3_control_w33]; the comparators run 48 to 80 [nw_ct388_obes, gf1_s1_design, rt_t1_wt]. At least four weekly incretins with ≥17% TRE or EE loss at 48–80 weeks will be on the market or filed before VK2735's modelled mid-2029 approval [A]: tirzepatide −20.9% TRE [gf1_s1_tre], retatrutide −25.0% TRE [rt_t1_wt], enicepatide −18.3 pp TRE placebo-adjusted [nw_ct388_obes], CagriSema −20.4% TRE [nw_cagri_r1_tp], ribupatide SC −17.7% TRE [nw_ribu_cn_p3]. CagriSema missed non-inferiority to tirzepatide head-to-head (EE 23.0% vs 25.5%) [nw_cagri_r4]: being close to tirzepatide does not win share. Every cell carries a tier (resolved / annotated / inferred / unknown). VK2735's own rows are Phase 1/2, ≤33 weeks and sponsor-reported.

Estimand
■ resolved · registry, paper, label, SEC filing ■ annotated · sponsor PR, conference, news ■ inferred · arithmetic, digitised or analyst ■ not disclosed / estimand not stated ■ other estimand (greyed)

EE = efficacy estimand (on-treatment). TRE = treatment-regimen / treatment-policy estimand (ITT-like; FDA generally asks for it). Viking's "eff-pop MMRM" is its efficacy population analysed with MMRM and sits closer to EE, so it is shown in EE mode as ≈EE. PA = placebo-adjusted percentage points; abs = change from baseline. VK2735 and retatrutide approval cells read the live timing sliders in section 10. CagriSema is a fixed-dose combination, shown for context but not monotherapy. The superiority claim, adjudicated

Matched timepoints only. Empty cells are drawn empty: no extrapolation, no proxy. Digitised values carry ±0.3 pp reading error.

Viking frames VK2735 17.5 mg at 22% placebo-adjusted at week 33 against "~14%" for approved GLP-1/GIP agonists and "~17%" for "the most advanced triple agonist", caveated as non-head-to-head [rel-06]. Which trials and weeks sit behind the 14% and 17% is not stated; ~14% is consistent with tirzepatide 15 mg at W33 (EE, placebo-adjusted ~−14.5) [gf2_tzp15_w33]. No opened source claims superiority verbatim [rel-18]. Below, each claim is set against the comparator at the same week, on the same estimand family (efficacy). VK2735 figures are Phase 1/2, ≤33 weeks, sponsor-reported.

Verdict

Superiority over tirzepatide is not established, but the early matched data lean towards VK2735.

  • At matched weeks 20–36, VK2735 17.5 mg shows ~3–5 pp more absolute loss than tirzepatide 15 mg: −17.8% vs −12.6% at W20–21, −21.7% vs ~−17.5% at W33 [gf3_ind_17p5, gf2_tzp15_w20, gf3_control_w33, gf2_tzp15_w33]. Placebo-adjusted the gap is ~7 pp, of which ~2.5–3 pp is the placebo arms: Viking's n=15 placebo moved −0.1% / +0.3%, SURMOUNT-1's −2.4% to −3.0% [gf3_ind_placebo, gf2_pbo_traj] [A].
  • Faster titration does not explain it alone. Tirzepatide 10 mg, after 8 weeks at its top dose, was at −12.7% at W20; VK2735 was at −17.8% after ~8 weeks at 17.5 mg [gf2_tzp10_w20, gf3_titration]. The gap persists once both drugs have had ≥13 weeks at top dose [rel-19, gf3_titration].
  • Against: n=13 at W33, one placebo arm of 15, a pooled n=118 cohort, weight exploratory and unregistered, a lighter and healthier population (95.9 kg vs 104.8 kg baseline), 13% early dropout for mostly undisclosed reasons, and nothing beyond W33 [gf3_control_w33, gf3_ind_pools, gf3_registry, gf3_ind_17p5, gf1_s1_design, gf3_ind_dc].
  • Against retatrutide: level at W21–24 (−17.8% vs −17.5%) [gf3_ind_17p5, rt_p2_wk24], with nothing to set against −26.1 pp EE placebo-adjusted at W80 [rt_t1_wt].

Defensible claim: an early, unconfirmed efficacy lead over tirzepatide, plus a credible monthly maintenance regimen. Not "better than tirzepatide".

What would change it. VANQUISH-1 17.5 mg at week 78 with TRE ≥ 20.9% [gf1_s1_tre] or EE ≥ 22.5% [gf1_s1_ee] and AE discontinuation ≤ 7% [gf1_s1_dcae] → bull. TRE < 17% or EE < 18.6%, or AE discontinuation > 12% → bear [A]. The primary estimand is undisclosed, so both are carried [gf4_estimand_status]. Even VANQUISH is a cross-trial comparison; no head-to-head trial is planned.

Tolerability — by dose and duration

All-cause and drug-related GI rates are labelled and never mixed: VENTURE-Oral reports drug-related GI only. Windows differ from 12 to 80 weeks.

Induction is where VK2735's tolerability is decided, and the topline headline hid it. Over 21 weeks the call deck shows nausea 42% vs 7% and vomiting 27% vs 7% on combined VK2735; at 22.5 mg, nausea 80% [gf3_ind_gi]. That replaces the CEO's spoken "5% range" [rel-01, superseded by gf3_ind_gi]. Only 1 of 166 stopped for an adverse event, but 22 of 166 (13%) left early, and the reasons for 21 of them are not disclosed [gf3_ind_dc]. VENTURE's 13-week fast titration ran to 20% AE discontinuation at 15 mg [vk_venture_aedc], against 6.2% for tirzepatide 15 mg over 72 weeks [gf1_s1_dcae] and 11.3% for retatrutide 12 mg over 80 [rt_t1_safety]. The 12-week maintenance window looks clean (nausea 4.5–5%, all mild; AE discontinuation 1/37 and 1/66) [gf3_maint_safety], but only patients who got through induction were in it. The oral is the weak spot: at 120 mg, 30% AE discontinuation and 38% any-reason over 13 weeks [vk_oral_aedc, vk_oral_anydc]. Its GI rates are drug-related, not all-cause, so they are kept apart from the SC bars [vk_oral_gi]. Windows run from 12 to 80 weeks; every bar is labelled with its own.

Bars carry their window. VK2735-102's 1% AE discontinuation is 21 weeks [gf3_ind_dc]; tirzepatide's 6.2% is 72 [gf1_s1_dcae]. VENTURE-Oral toggles AE vs any-reason discontinuation (30% vs 38% at 120 mg) [vk_oral_aedc, vk_oral_anydc]. Its vomiting bars are drug-related and drawn in a different pattern from all-cause SC rates [vk_oral_gi]. Bar scales: discontinuation and vomiting 0–40%, nausea 0–80%. Maintenance economics

What is genuinely new is monthly maintenance. The calculator drives the live model: moving it moves every number on the page.

After 21 weeks of weekly dosing, Viking re-randomised patients for 12 weeks. Every-other-week dosing kept 90% of the week-21 loss, monthly 85%, and switching to placebo still kept 61% [gf3_maint_qow, gf3_maint_qm, gf3_maint_placebo]. Read the last number first: when stopping the drug altogether keeps 61% over 12 weeks, a 12-week window flatters any regimen. In absolute terms monthly maintenance lands at ~15.1% below baseline against 21.7% on continued weekly dosing [gf3_implied_w33, gf3_control_w33]. Viking does not define "% maintained", and one excluded outlier moves an 11-person arm from 99% to 85% [gf3_maint_def, gf3_outlier]. Different windows: tirzepatide's 5 mg step-down kept ~77% over 52 weeks [gf1_sm_ret5] and orforglipron after tirzepatide 74.7% [or53, nw_attain_maintain]. These are not like-for-like with the 12-week VK2735 figures. The value question is price, not retention. If payers pay per month, a monthly patient-month is worth ~0.85 of a weekly one; if they pay per pen, ~0.23 [A]. No obesity drug has a frequency-based price precedent [unverified: gf10_ratio_freq]. The market has already paid for monthly dosing once, in Pfizer's Metsera CVRs [nw_mtsr_final].

Calculator · maintenance mix × maintenance price

Blended SC net price = launch price × (1 − maintenance share + maintenance share × price ratio). Units per patient-year = weekly units × (1 − share) + monthly units × share. "Per pen" sets the ratio to monthly ÷ weekly units (~0.23), floored at the slider's 0.25 minimum (HANDOFF §6.1); "per month" restores the active case.

The visual evidence

Revenue fan, stress grids, value waterfall and an in-browser Monte Carlo.

a · Revenue fan, stacked by line

2034 unrisked: bear $0.19B, base $1.71B, bull $9.61B. Risked: $0.13B / $1.38B / $8.16B (§7). The bull case is 69% of the scenario-weighted risked mean ($2.36B), so the median outcome sits nearer the base than the mean [A]. T2D and ex-US are 35% of base unrisked revenue (591 of 1,713 $M) on no VK2735 data in either [vk_t2d] [A]. Bars: the active case stacked by line; lines: bear, base and bull totals.

b · VK2735 SC share × launch net price

Risked peak revenue, $B. Base 5% at $300 a month [A]. Late entrants without outcomes data or scale have taken 0–2% of their class (Steglatro 0.2%, alogliptin 1.4%) [mk42, mk43]; Mounjaro took 23.4% of T2D incretin claims with both [mk45]. Read the grid before trusting a single-point peak. The outlined cell is closest to the live sliders.

b · Retatrutide share × maintenance mix

rNPV per diluted share, $. Retatrutide 20% of the pool in base, 30% in bear [A]. TRIUMPH-5 (primary completion 2026-11, est.) moves this axis first [rt_t5]. Rows: share of SC patient-months on QOW/QM maintenance. The outlined cell is closest to the live sliders.

Which lever moves the share price?

One-at-a-time sensitivity. For each lever, every other input stays at the active case and only that lever is moved across its bear-to-bull range. The bar spans the resulting rNPV per share: red to the left of the base tick is the downside if the lever lands at its unfavourable end, blue to the right is the upside at its favourable end. The value at each end is printed on the bar; the small text under the lever name gives the unfavourable and favourable lever settings in that order. Right column: total swing, then downside / upside from base. Rows are ranked by total swing. This is not a confidence interval: it is the price impact of one input at a time.
