Two degraders, no disease efficacy yet — gout has to prove MRT-8102, and MRT-6160 is a milestone stream Novartis controls.

MRT-8102 cuts hsCRP by the same ~80–85% the oral NLRP3 class reports; after ZEUS that carries no CV value on its own, and the first disease-level test is a ~60-patient gout-prevention study reading out in H2 2027. MRT-6160 has seven days of healthy-volunteer dosing behind it; Novartis runs, funds and can end its two Phase 2s, which read out around Dec 2028 and Feb 2029. About 85% of its ~$185M base value to Monte Rosa is milestones. At $11.26 the market prices ~$0.37B for MRT-8102, above our $0.02B base. The thesis, in one picture

Risked MRT-8102 revenue by indication for the active scenario, with Monte Rosa's risked MRT-6160 receipts (not Novartis's sales) as a separate line. Dashed verticals are catalysts.

GFORCE-1 showed what a NEK7 degrader does to blood markers in four weeks: hsCRP down a median 85% in treated subjects with week-4 data, within the ~78–86% the oral NLRP3 class reports, with the placebo arm also falling [dk_pd_hscrp, cp_class_hscrp_effect] [unverified: as_placebo_hscrp]. It did not test a disease. The first disease-level test is GEMINI-1, a ~60-patient gout-prevention study with initial data in H2 2027 [as_gemini1]; gout carries the largest base rNPV, and CAD only works if a partner pays for an outcomes trial (§6.2). MRT-6160 is a licence: Novartis funds and runs development, decides Phase 3, owns the lead Sjögren's asset ianalumab and may terminate for convenience [vv12, vv13, vv37]. Monte Rosa's base value from it is mostly milestones [A].

Verdict and evidence ladder

One column per asset. No composite score: each rung is a separate question, and "not established" is drawn empty.

Verdict, in plain words

MRT-8102: a potent, low-dose, oral inflammasome-pathway drug with class-level CRP effects and an untested breadth hypothesis. It is not shown to be a better NLRP3 drug, and it is not a CV drug. Strong human target-engagement and PD evidence for an early asset; clinical benefit unproven [dk_nek7, dk_pd_hscrp, cp_class_hscrp_effect].

MRT-6160: strong blood target engagement and ex vivo pathway suppression after seven days in healthy volunteers; no patient data; its value to Monte Rosa is a contract Novartis controls [vv19, vv20, vv18, vv13].

03

GFORCE-1 design and baseline

Phase 1 (Part 3 of the first-in-human study), four weeks of dosing, sponsor-reported. The placebo arm started with lower hsCRP than every active arm.

Design

Randomised 3:1 within each dose cohort, so the n=27 placebo arm pools three cohorts. Not an efficacy trial: the primary endpoint is safety and tolerability.

Baseline hsCRP by arm: median and IQR, mg/L

Placebo 4.8 vs 5.6–6.4 mg/L in active arms. Entry required hsCRP >3 mg/L (dashed line), so regression to the mean is expected in every arm and within-arm % changes are partly regression. Median age 41: a young, obese population, not a CAD population [dk_baseline].

04

Target engagement: NEK7, blood only

A band, not per-arm values: GFORCE-1 arm medians are not disclosed. Peripheral-blood T cells only.

~80–90% NEK7 degradation in peripheral-blood T cells

Medians with IQR in the deck across SAD (n=48), MAD (n=40) and GFORCE-1 week 4 (n=101), at all three GFORCE-1 doses [dk_nek7]. Not measured: monocytes and macrophages, plaque, synovium, HS lesions. Blood T-cell engagement is not tissue engagement. The rabbit gout model reduced swelling at only ~40% degradation [as_preclin_models].

05

Pharmacodynamics at week 4, by biological layer

Median within-arm % change from predose in MRT-8102-treated subjects with week-4 data (n=74 of 81). Not placebo-adjusted. Unknown placebo values are drawn empty, never as zero.

Every marker fell; the hsCRP placebo arm fell too

The analysis set is subjects with week-4 data, n=74 (27 / 26 / 21 by dose); the deck does not call them completers, and the 7 excluded include AE discontinuers, 5 of them at 40 mg, so results may favour tolerators [as_rx_conditions]. Placebo hsCRP read off a figure: ~−16% week 1, −15% week 2, −25% week 3, −28% week 4, ±3 pts [unverified: as_placebo_hscrp]; placebo calprotectin (~−4%) and fibrinogen (~0%) barely moved [as_placebo_other], consistent with regression to the mean on the hsCRP entry criterion.

Table: PD values with set, n, p and source
06

Dose level: a plateau at the lowest dose

No monotonic dose-response on any disclosed marker. AE discontinuations rise with dose on the same axis.

hsCRP and calprotectin by dose, with AE discontinuations

Consistent with saturation at ≤5 mg, matching the SAD/MAD NEK7 plateau [as_barclays_dose]. It does not show that 5 mg is optimal: no lower dose was tested in GFORCE-1 and arm-level NEK7 is undisclosed [dk_nek7]. Bars: median % change (left axis, not placebo-adjusted). Red: AE discontinuations of 27 per arm (right axis) [dk_pd_hscrp, dk_pd_calpro, dk_safety_arms].

The numbers

Interim vs final. The January interim (40 mg only, 24 subjects, active and placebo pooled, split undisclosed) reported −85% and 94% below 2 mg/L [as_interim_40mg]. The final pooled rate is 86% of treated subjects with week-4 data (n=74, three doses) [dk_pd_hscrp]. The two are not comparable. GFORCE-2 doses are 5 / 10 / 15 mg; 20 and 40 mg are dropped [as_gforce2_oct]. The call transcript that may explain why is paywalled and was not read [unverified: as_call_unavail].
07

Safety by arm, 8 weeks of observation

The press release's pooled "33% vs 30%" hides a dose gradient (26 / 30 / 44%) and omits the 8 discontinuations.
View

Adverse events, % of arm (n=27 each)

Counts and discontinuation detail

Four weeks cannot establish chronic safety. Long-term (6-month-study-enabling) toxicology is not complete [as_chronic_tox]. NEK7 cell-cycle biology is an open question: no fact in the base addresses it. Headache 4% vs 11% placebo; arthralgia 5% vs 4% [dk_safety_pooled].

Verdict: not shown to be differentiated on today's data

        • MRT-8102 lowers hsCRP by the same ~80–85% the oral NLRP3 class reports. The only like-for-like duration (28 days, ruvonoflast) shows no measurable gap once placebo is considered, and MRT-8102's own placebo figure is readable only off a chart [dk_pd_hscrp, cp_ruvonoflast_jacc] [unverified: as_placebo_hscrp].
        • Distinctive but unproven: (a) a dose plateau at 5 mg with clean 5 mg tolerability on 27 subjects; (b) broad DAMP suppression (calprotectin, S100A12, SAA) that comparators have not reported. Neither is a clinical advantage until a disease endpoint moves [dk_safety_arms, dk_pd_calpro, dk_pd_s100a12_saa].
        • Against colchicine and IL-1/IL-6 biologics the question cannot yet be asked: outcome trials show CRP lowering marks pathway engagement, not benefit, and MRT-8102 has no outcome, imaging or disease data [cp_zeus, cp_clear_synergy].

Defensible claim: a potent, low-dose, oral inflammasome-pathway drug with class-level CRP effects and an untested breadth hypothesis.

        Disease yardsticks and deal context

What GEMINI-1 and GALAXY-1 have to beat

          NLRP3 deals: context, not inputs

Lilly paid ~$1.2B equity for two Phase 2 NLRP3 assets with biomarker data only [cp_ventyx_lilly_value]. GLUE's EV is ~$0.47–0.52B [co_mcap]. A reference line on the sum-of-parts chart, never a valuation input.

MRT-2359 is Monte Rosa's oral GSPT1 degrader, given with enzalutamide in metastatic castration-resistant prostate cancer (mCRPC). An investor note in circulation says the market ignores it. The data show a real-looking signal, but in five patients with androgen-receptor ligand-binding-domain (AR-LBD) mutations, identified after the fact, with no control arm and PSA as the main readout [gs04, gs08, gs37]. The base stays at $50M [A] and the base value per share at $8.42; only the bull moves, from $150M to $250M [A]. The niche is about 400–950 US post-Pluvicto patients a year [unverified: pc_31], and that will not move the needle.

                  Method, self-tests and caveats

Analysis set and placebo rules. Every GFORCE-1 PD number is a median within-arm change from predose in MRT-8102-treated subjects with week-4 data (n=74 of 81; 27 / 26 / 21 by dose), not placebo-adjusted [dk_pd_hscrp, as_rx_conditions]. The placebo hsCRP trajectory is read off a figure with no numeric label and is shown only with an [unverified: as_placebo_hscrp] tag. HMGB1 and IL-1β are elevated-baseline subgroups (75th percentile of N=101; n=20 and n=17; p=0.05 and 0.003) [dk_pd_subgroups]. "Comparable with healthy volunteers" refers to a separate, non-concurrent healthy-volunteer cohort [dk_hv_comparator]. Lp(a) −24% is press-release only [as_lpa_pr].
MRT-8102 rNPV. Per indication: peak US = pool × layers × share × fraction of year on therapy × list × (1 − gross-to-net); ex-US = US × ex-US ratio with a one-year lag, always out-licensed at the ex-US royalty [A]. Revenue ramps linearly to peak over the ramp years, falls to 20% in the LoE year and 10% after, and loses the IRA cut from year 9 after launch [mk25]. Stage gates p2 × p3 × approval; Phase 2 cost 2027–28 at probability 1; Phase 3 cost spread over the n3 years ending the year before filing, × p2 (CAD 5 years); launch build in L−1 (× p2·p3), L and L+1 (× cumulative PoS); shared programme cost $60M over 2027–28. Partner mode 1 (base) partners CAD after Phase 2: the partner pays Phase 3 and commercial, Monte Rosa gets a royalty and milestones (2/3 at Phase 3 start × p2, 1/3 at launch × cumulative PoS). Tax on positive risked contribution less build (NOLs ignored). No floor at zero: an indication can be negative because its Phase 2 is committed; the exclude toggle is the abandonment option. Discounting (1+r)^(t−2026), horizon 2026–2050.
MRT-6160 module. Peaks from Sjögren's (Novartis 353K US diagnosed [vv34]) and PsA (0.05–0.25% prevalence [vv45]) funnels; ex-US × m6_exus with a one-year lag; own LoE m6_loe = 2045 [A] (VAV1 expiry not found). Joint outcome tree: Sjögren's Phase 2 positive m6_sjo_p2; PsA positive given Sjögren's positive / negative m6_psa_p2_if / m6_psa_p2_ifnot; if ≥1 positive, Novartis continues with m6_nvs_cont, else terminated (value zero thereafter); Sjögren's goes to Phase 3 with m6_sjo_prio (the ianalumab overlap); each indication in Phase 3 fails, files but is not approved, or is approved; probabilities sum to 1, checked in code. Cash flows: PsA Phase 2 milestone × its probability in 2027; first Phase 3 start (L−5); first US approval (L); first ex-US approval (L+1); second-indication approval; six sales milestones on annual ex-US net sales (worldwide if opted out); tiered ex-US royalties (breaks $250M / $500M / $1B). Opt-out: tiered US royalties, no Phase 3 cost, no P&L. Co-fund: − m6_cost_share × Phase 3 cost over L−4..L−2, + 30% × (US sales × (1 − COGS − SG&A) − launch cost). Tax on royalties, P&L share and sales milestones; dev/reg milestones untaxed [A]. The $50M first Phase 2 milestone is cash in the bridge, never in the module. The milestone envelope (dev/reg incl. the paid $50M + sales ≤ $2.1B; sales ≤ ~$0.6B; dev/reg > $1.5B once multiple indications are approved) is enforced in code [vv03, vv04, vv05].
Bridge, TSM and dilution. Equity = MRT-8102 rNPV + MRT-6160 value + platform + MRT-2359 − PV of unallocated corporate G&A (corp_ga × annuity) + net cash est. 2026-09-30 (cash 6/30 + $50M milestone − Q3 burn) + raise proceeds net of fees [co_cash_total, co_nvs24_ms1, co_burn]. Future dilution is modelled as shares, not a dollar deduction: the raise issues raise_amt / raise_px shares with raise_px = spot × (1 − raise_discount), so it moves with the quote. Options use the treasury-stock method on the weighted-average exercise price ($9.18), solved in closed form; below $9.18 they add nothing [co_options_rsu]. Per share = equity / (basic + PFW + RSUs + new shares + TSM options) [co_pfw]. When the issue price is above intrinsic value per share, the raise adds value per share; when below, it costs value.
